SAMUEL
16 Years Old
ID#  WKMB3534

Samuel is a creative boy who loves arts and crafts, drawing and coloring. He enjoys building things with his legos, swimming and basketball. He is friendly, playful with others and enjoyable to be around. He is able to entertain himself for extended periods of time if he is participating in an activity he enjoys such as drawing and coloring. Samuel is in a special education classroom due to his developmental delays and mental retardation. He enjoys going to school and was student of the month two times last school year. Samuel has been diagnosed with bipolar disorder, developmental delays, fetal alcohol syndrome, mental retardation and pervasive developmental disorder. He also has a history of seizure disorder, but has not had seizures in several years. He also has the hemophilia trait, but does not have active symptoms. A specialist for this blood disorder monitors Samuel at least yearly. Samuel needs a loving, but structured family
